You will provide Urgent Community Response services to patients experiencing health crises, supporting them in their homes across Wandsworth borough, with occasional travel to Merton. The service operates from 8am to 8pm, 7 days a week.
The role involves establishing, coordinating, and delivering diverse, evidence-based, and innovative clinical care. You will develop your clinical skills and professionally support primary and secondary care to prevent hospital admissions by facilitating integrated care in patients' homes.
Key responsibilities include making evidence-based clinical decisions, undertaking clinical interventions, maintaining communication with the team, conducting handovers, supervising junior staff, prescribing and managing equipment, maintaining accurate records, and collaborating with other professionals and voluntary agencies.
Our services support over two million people across eleven London boroughs and Hertfordshire, providing high-quality healthcare at home and in clinics, helping patients stay well, manage their health, and avoid unnecessary hospital visits.
