Job summary

Royal Devon University Healthcare NHS Foundation Trust is seeking an Urgent Community Response Support Worker. You will join a dedicated team delivering care at patients' homes, supporting in and out of hospital settings and promoting independence.

The role includes 30 hours per week with varied shifts, across Sidmouth, Seaton and Axminster areas. We value inclusivity and offer training, development and NHS benefits.

Qualifications

• Level 3 Diploma / NVQ3 in Health and Social Care or equivalent.
• Previous community experience preferred but not essential.
• Competent with IT and electronic recording.
• Valid driving license and access to a vehicle.

Responsibilities

• Deliver personal care and promote nutritional needs.
• Assist in recovery and discharge to support independence.
• Work within a multi-disciplinary team in community settings.
• Record care activities using electronic systems.
