Job summary

Northamptonshire Healthcare Foundation Trust (NHFT) in the UK is seeking dynamic community nurses to support the Urgent Community Response Service. You will help keep patients out of hospital by delivering care in their homes in collaboration with Therapists, Doctors and support workers.

Qualifications

• Valid UK driving license and access to a vehicle.
• Flexible to work 12.5h and 7.5h shifts, including weekends and bank holidays.
• Experience delivering nursing care in patients' homes and IV therapy.
• Ability to work in an integrated, multi-disciplinary team with Therapists and Doctors.

Responsibilities

• Assess the healthcare needs of patients and families.
• Monitor the quality of care they are receiving.
• Be professionally accountable for the delivery of care.
• Utilise technology to update records as you go.
