Job summary

Shropshire Community Health NHS Trust seeks a Staff Nurse for the Bridgnorth locality team of its Urgent Community Response and Virtual Wards service, with bank-only work also available. The post involves assessing acutely unwell patients at home and delivering agreed care plans as an alternative to hospital admission, using visits and digital technology.

It runs seven days a week and provides access to a multidisciplinary team including consultants, GPs and pharmacists.

Qualifications

• NMC registration is required and must be current.
• Experience in community nursing preferred.
• Ability to document care notes clearly.

Responsibilities

• Assess acutely unwell patients at home and implement agreed care plans.
• Deliver holistic, evidence-based nursing assessments and care.
• Combine face-to-face visits with digital technology for care.
• Collaborate with doctors, social workers, therapists, and GP services.
• Provide a seamless service to avoid unnecessary hospital admissions.
• Work within a multidisciplinary team including consultants and pharmacists.
• Help develop the Urgent Community Response service under the NHS Long Term Plan.
