Greater Peterborough Network Ltd in the United Kingdom seeks an experienced Healthcare Support Worker for the Urgent Community Response and Virtual Ward teams. The post holder will visit patients in their own homes to undertake observations, venupuncture, wound reviews and other assessments, supported by registered clinicians.

The role includes coordinating remote monitoring, office-based calls, and ensuring timely, compassionate care.

Formación

• Literacy and Numeracy (level 2) or equivalent e.g. Maths/English GCSE grade C or above/willing to work towards and achieve within 6 months of commencing
• Diploma Level 2/3 in Care/Health or other relevant qualification

Responsabilidades

• Visit patients at home as part of a team and provide patient care and administrative duties as directed by a registered practitioner
• Deliver a high standard of holistic care, promoting equality and dignity at all times
• Maintain confidentiality and be polite at all times
• Communicate effectively with the team to coordinate appointments
• Assist with preparation, cleanliness and tidiness of all equipment and complete equipment audits
• Ensure professional interactions with patients in person and on the telephone
• Accurately enter patient details on all digital systems
• Support chaperone policies and remote monitoring observations
