Job summary

Livewell Southwest is seeking a Health Care Assistant to support the Acute Virtual Ward in patients' homes across Plymouth, South Hams and West Devon. This part-time role (up to 37.5 hours per week, 7.5-hour shifts) is ideal for an experienced and motivated health care assistant eager to contribute to a growing Urgent Care Nursing service.

Qualifications

• NVQ Level 3 in Health and Social Care or equivalent qualification.
• Qualified to NVQ Level II or able to demonstrate the skills.
• Phlebotomy qualification or experience is beneficial.
• Experience in community health care settings and home visits.

Responsibilities

• Deliver high quality clinical care for patients in their homes.
• Assess, monitor and escalate concerns using SBAR.
• Support care planning and record keeping for patient records.
• Travel between Livewell Southwest sites and community locations.
• Work across a 7 day service with flexible shift patterns.
