Location: Worcestershire, WR5 2PN

Permanent / Fixed Term Contract

Lead Outstanding Practice. Deliver Home First. Change Lives.

Are you an experienced and inspiring leader with a passion for helping people achieve the best possible outcomes when they need health and social care support most?

We are seeking an exceptional Urgent Care Team Manager to lead our Community Hospital-based service across Worcestershire. This is a unique opportunity to play a pivotal role in delivering our Home First vision by ensuring people receive the right support, in the right place, at the right time, enabling them to maximise their independence and avoid unnecessary reliance on long-term services.

Working across Worcestershire's Community Hospital estate, you will lead a skilled and dedicated team responsible for supporting people through hospital discharge pathways, promoting recovery, rehabilitation and independence. You will be a key partner in our integrated approach alongside NHS colleagues, providers and wider Adult Social Care teams, helping to ensure people can return home safely and successfully wherever possible.

About the Role

As the Urgent Care Team Manager, you will provide leadership, professional oversight and operational management for a multidisciplinary service delivering assessment, recovery-focused interventions and discharge support within Community Hospital settings.

This role sits at the heart of Worcestershire's Home First approach, championing strengths-based practice and ensuring every intervention begins with the question:

"What matters to this person, and how can we support them to remain as independent as possible?"

You will:
  • Lead and develop a high-performing team delivering assessment and support across Worcestershire's Community Hospitals.
  • Drive the delivery of Home First and Discharge to Assess principles, ensuring people are supported to recover in the least restrictive and most independent setting possible.
  • Foster excellent partnership working between Adult Social Care, NHS colleagues, providers and community services.
  • Promote high-quality, strengths-based practice that focuses on people's abilities, networks and recovery potential.
  • Support the reduction of unnecessary hospital stays and long-term care dependency.
  • Drive quality, performance and continuous service improvement.
  • Provide leadership, supervision and professional support to staff, creating a positive and empowering culture.
  • Work closely with the Urgent Care Team Manager covering Acute Hospitals to ensure a coordinated and seamless approach across Worcestershire's hospital discharge pathways. You will support each other's operational and strategic responsibilities, collaborate on service developments and improvement initiatives, and provide leadership cover during periods of annual leave, absence or increased service demand, ensuring continuity of management and decision-making across both hospital-based services.
Why Join Us?

This is more than a management role. It is an opportunity to shape the future of urgent care and hospital discharge services across Worcestershire.

You will have the chance to:

  • Influence service delivery across the county's Community Hospital network.
  • Lead a service that makes a tangible difference to people's lives every day.
  • Be at the forefront of integrated working between health and social care.
  • Help deliver Worcestershire's Home First ambitions and transformation agenda.
  • Work with passionate colleagues who are committed to innovation, collaboration and continuous improvement.
Why Worcestershire?

This is an exciting time to join Adult Social Care in Worcestershire. We are continuing to strengthen our Home First approach, working with health and care partners to support people to live as independently as possible and achieve the outcomes that matter most to them. As a senior leader within Urgent Care, you will have genuine opportunities to influence strategy, shape services and develop innovative solutions that improve outcomes for residents while supporting system-wide performance.

If you are a collaborative, values-driven leader who thrives in a dynamic environment and is passionate about delivering person-centred care, we'd love to hear from you.
