Job summary

Kent Community Health NHS Foundation Trust is seeking an HCPC-registered Occupational Therapist or Physiotherapist to join its urgent care service across community settings. You will assess, plan, implement and evaluate holistic care to help patients remain at home and support timely discharge.

You will work autonomously within a multidisciplinary team, manage complex caseloads, participate in triage, and contribute to innovations such as Virtual Wards and remote monitoring to improve patient

Qualifications

• HCPC-registered Occupational Therapist or Physiotherapist.
• Experience in urgent care, rehabilitation and community settings.
• Ability to work autonomously within a multidisciplinary team.

Responsibilities

• Assess, plan, implement and evaluate holistic care for patients with complex needs.
• Manage own caseload, undertake triage and urgent assessments.
• Identify and respond to clinical deterioration and coordinate equipment provision.
• Contribute to manual handling plans and patient safety initiatives.
• Support innovative models of care including Virtual Wards and remote monitoring.
