Working across community settings, you will assess, plan, implement and evaluate holistic care for patients with complex needs, promoting independence through rehabilitation and enablement. You will manage your own caseload, undertake triage and urgent assessments, identify and respond to clinical deterioration, prescribe appropriate treatment interventions, coordinate equipment provision and contribute to complex manual handling plans.
