Kent Community Health NHS Foundation Trust is seeking an Occupational Therapist or Physiotherapist to join the Urgent Care Service in the community. The role involves assessing people with complex needs and delivering rehabilitation to support independence and discharge from hospital.

You will work across community settings, travel as required, and maintain HCPC registration and a full UK driving licence. The post supports Virtual Wards and quality improvement initiatives.

Formación

• HCPC registration as an Occupational Therapist or Physiotherapist.
• Full UK driving licence.
• Right to work in the UK.
• Experience mentoring/supervising health staff or students.

Responsabilidades

• Assess, plan, deliver and review holistic care for patients with complex needs in community settings.
• Triage referrals and carry out urgent assessments.
• Manage a caseload to promote independence through rehabilitation and enablement.
• Provide specialist advice and support colleagues with complex assessments and clinical decisions.
• Support Virtual Wards and care at home through remote monitoring.
• Contribute to service development and quality improvement objectives.
• Travel across community settings to deliver care.
