Job summary

Practice Plus Group is seeking experienced Advanced Nurses or Clinical Practitioners to join our Out of Hours team across Warwickshire, including Nuneaton, Coventry, Rugby, Warwick, Stratford-upon-Avon and Shipston-on-Stour. The OOH service operates 18:30–08:00 Mon–Fri and 24 hours on weekends, with flexible session patterns to suit you.

You will assess and treat urgent unscheduled needs, supported by a multidisciplinary team. DBS and references required; equal opportunity employer.

Qualifications

• Registered Nurse or Paramedic with MSc in Advanced Clinical Practice.
• Experience in Out of Hours, Primary Care GP Surgery, or Urgent Treatment Centre.
• Holder of V300 / Non-Medical Prescribing qualification.
• Ability to see and treat patients of all ages autonomously.
• Excellent communicator with a patient-centred approach.
• Flexible, self-motivated and committed to ongoing professional development.

Responsibilities

• Deliver comprehensive clinical assessment, diagnosis, and treatment for urgent needs.
• Conduct consultations from the clinical base with face-to-face care.
• Undertake home visits to patients requiring assessment or treatment in their own residence.
• Assist with telephone triage when there are fewer face-to-face consultations booked.
• Manage telephone triage, providing clinical advice, risk assessment, and directing patients to care pathways.
• Work autonomously while collaborating with a multidisciplinary team to ensure patient-centred care.
• Maintain accurate clinical records and adhere to NHS guidelines.
