Job summary

Kent Community Health NHS Foundation Trust is seeking a Community Staff Nurse to join our West Kent Urgent Care Service as a Twilight Nurse, delivering rapid crisis response to help patients stay at home and avoid unnecessary hospital admissions.

You will work across a 24/7 service with a wide range of clinical skills, including IV therapy, syringe driver management, catheterisation, phlebotomy, wound care and clinical assessment, within a supportive multidisciplinary team.

Responsibilities

• Undertake comprehensive assessments, develop and review personalised care plans, and work in partnership with patients, families and wider professionals to achieve the best outcomes.
• Coordinate care, arrange equipment, make onward referrals and deliver a range of clinical interventions while monitoring patients for signs of deterioration and responding appropriately.
• Manage a daily caseload, support triage and prioritisation of referrals, and contribute to the smooth running of the service.
• Supervise and support healthcare assistants, students and junior colleagues.
