Working across a 24/7 service, you'll deliver compassionate, responsive care to patients with urgent, complex and end-of-life needs. Using a wide range of clinical skills, you'll provide treatments often associated with hospital care, including IV therapy, syringe driver management, catheterisation, phlebotomy, wound care and clinical assessment.
This is a rewarding opportunity for a confident and caring nurse who enjoys autonomous working while being part of a supportive, experienced multidisciplinary team. You'll collaborate closely with health and social care colleagues to deliver high-quality, patient-centred care that has a direct impact on people's lives during times of crisis.
Please note, this vacancy does not meet the criteria for skilled worker sponsorship. Therefore, we are unable to accept applications from candidates that cannot provide documentary evidence of right to work in the United Kingdom.
As a valued member of our community nursing team, you'll provide holistic, person-centred care to patients with diverse and often complex healthcare needs.
You’ll undertake comprehensive assessments, develop and review personalised care plans, and work in partnership with patients, families and wider professionals to achieve the best outcomes. You’ll coordinate care, arrange equipment, make onward referrals and deliver a range of clinical interventions while monitoring patients for signs of deterioration and responding appropriately.
The role also includes managing a daily caseload, supporting triage and prioritisation of referrals, and contributing to the smooth running of the service. You’ll play an important part in developing others by supervising and supporting healthcare assistants, students and junior colleagues.
This role is ideal for a proactive nurse who thrives in a community setting, values teamwork and is passionate about delivering outstanding care closer to home.
