Salary: Agenda for Change Band 7 £51,488 - £57,802 pa pro rata, dependent on experience (includes HCA Supplement)

It is an exciting time at Trinity as we launch our new community services, which make up our new clinical hub. The new services have been developed to streamline the coordination of care for patients, address the evolving palliative and end of life care needs of our local population, and maximise the flexibility and responsiveness we can offer.

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of Triage Clinical Nurse Specialist within our First Contact team.

This role requires you to act as the first point of clinical contact for patients following referral, identifying and responding to immediate specialist palliative care needs through direct liaison with patients, carers, or other involved health and social care professionals.

Key responsibilities
  • Lead in receiving and managing referrals made to the hospice.
  • Triage all incoming referrals considering patient’s symptoms to determine appropriate level of care to safely meet their specialist palliative care needs.
  • Utilise resources and guidelines to assess, prioritise, advise, schedule care management, or refer patients to other appropriate agencies.
  • Ensure a timely, responsive, and effective service for all stakeholders involved in patient referrals.
  • Manage all aspects of the referral system, including appropriateness, urgency, and priority level.
  • Collaborate with internal and external partners to support patient care coordination and ensure care plans meet individual needs.
  • Use assessments and interventions as opportunities to train, coach, and mentor clinical staff and visitors on specialist palliative care practices.
Skills and experience required
  • Registered Nurse with bachelor’s degree in palliative/end of life care or related field and extensive experience in specialist palliative care.
  • Advanced assessment skills.
  • Critical thinking skills.
  • Knowledge of disease trajectories and treatment effects.
  • Advanced communication skills.
  • Enthusiasm and flexibility.
  • Ability to work autonomously and as part of a team.
  • Ability to travel across the catchment area (a UK driving license is desirable).
