Job summary

Equitas Medicare Group in the UK is seeking an experienced and compassionate Support Worker to join our supported living service, supporting adults with learning disabilities, autism and complex mental health needs.

You will help residents with daily living tasks, contribute to wellbeing sessions, and work with health and social care professionals to deliver person-centred care in a calm, respectful environment.

Qualifications

  • Ability to support adults with learning disabilities, autism and complex mental health needs.
  • Commitment to person-centred care and safeguarding.
  • Effective communication and teamwork with health professionals.

Responsibilities

  • Support residents with daily living tasks including cooking, budgeting, personal care, appointments, and maintaining a safe living environment
  • Develop and review person-centred support plans alongside residents and relevant professionals
  • Encourage independence, confidence, and community engagement
  • Maintain accurate records, daily notes, and incident reports
  • Deliver 1:1 and group wellbeing sessions focused on emotional regulation, resilience, coping strategies, and self-esteem
  • Support residents to identify goals and build structured wellbeing plans
  • Use trauma-informed and psychologically informed approaches in everyday support
  • Monitor wellbeing and respond appropriately to concerns, incidents, and safeguarding matters
  • Manage risk in a way that balances safety, dignity, and independence
  • Follow safeguarding, health & safety, and organisational procedures at all times
  • Work collaboratively with mental health services, social workers, GPs, and other professionals
  • Attend team meetings, reviews, and reflective practice sessions
  • Contribute to a positive and supportive team culture
