Job summary

Social Interest Group is seeking a dedicated Support Mentor to work in Kensington and Chelsea. You will assist residents in our supported accommodation, many facing homelessness and complex needs, including mental health, substance use and offending histories.

You will develop tailored support plans, run one-to-one and group sessions, coordinate multi-agency work, and help residents build skills for independent living in a trauma-informed, empowering environment.

Qualifications

  • Previous professional or lived experience supporting people who have multiple and complex needs.

Responsibilities

  • Support residents who may be facing challenges with homelessness, mental health, substance use, or feeling independent
  • Create tailored support plans through one to one sessions, group sessions, and multi agency meetings
  • Support residents to build skills and confidence to support move on to long-term independent living
  • Monitor residents’ wellbeing, carrying out regular welfare checks, and responding to any incidents or emergencies.
  • Plan and deliver activities to engage residents with other people, communities, and opportunities. This may include days out the service and in service activities
  • Keep support plans updated on our online portals as appropriate
