Job summary

Islington Council's Bright Start 0-5 Family Help Service is seeking a Family Help Family Support Practitioner to deliver early intervention and whole-family support. You will assess needs, hold a caseload and work with children, parents and partner agencies to provide timely help.

Based in London, you will visit families in home and community settings, coordinating with schools, health and local services to build resilience and prevent escalation of need, within a trauma-informed,

Qualifications

• Experience holding a caseload of families.
• Ability to undertake whole-family assessments.
• Develop outcome-focused family plans with partners.

Responsibilities

• Provide timely, flexible and responsive support to children, young people and families.
• Hold and manage a caseload and deliver home visits, community-based interventions, and work with Family Hubs and Children’s Centres.
• Collaborate with education, health, social care and voluntary sectors to coordinate support and monitor progress.
