Job summary

Eden Brown Synergy is recruiting a Social Worker in Bracknell to undertake Care Act assessments for youth transitioning to adulthood. You will develop person-centered plans, promote independence, and work with families, carers, and professionals across education, health, and social care.

The role requires a recognised Social Work qualification and registration with Social Work England, with flexible hours and a competitive referral bonus.

Qualifications

• Experience undertaking social care assessments and developing person-centred support plans.
• Knowledge of the Care Act 2014 and eligibility criteria for Adult Social Care.
• Strong communication and multi-agency working skills.

Responsibilities

• Undertake Care Act assessments for young people transitioning to adulthood.
• Develop person-centred care and support plans that promote independence and control.
• Collaborate with young people, families, carers and professionals across education, health and social care.
• Liaise with SEN teams, schools and colleges to plan for adult social care needs.
• Contribute to multi-agency planning to ensure coordinated transition into adult services.
