Qualifications

• A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
• Registered with Social Work England.
• Proven experience in social work, particularly within transition services.
• Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
• Excellent organizational skills and the ability to manage a caseload effectively.

Responsibilities

• Conduct assessments to identify client needs and develop tailored support plans.
• Providing direct support and guidance to clients and their families during transition periods.
• Coordinating with healthcare providers, educational institutions, and community resources to facilitate smooth transitions.
• Monitoring client progress and adjusting support plans as necessary to meet evolving needs.
• Documenting case notes and maintaining accurate records in compliance with legal and organizational standards.
