Job summary

Eden Brown Synergy is seeking a Social Worker for the Adults Transition Team in Bristol. The role offers excellent rates up to £32ph on a 3-month initial contract with flexible hybrid working. 37 hours per week.

The Young Adults Transitions Service supports ages 14–25 to maximize independent living as they reach adulthood. You will complete Care Act assessments, provide occupational therapy support, and help develop plans to meet needs, while coordinating with non-commissioned sources of support

Qualifications

• Registered with Social Work England and eligible to practice in the UK.
• Driver with access to own car preferred to support travel across the city.
• Flexibility for hybrid working in Bristol.

Responsibilities

• Conduct Care Act assessments for young adults aged 14–25 to support independent living.
• Provide occupational therapy support and assist in developing support plans.
• Assist in commissioning services and liaising with non-commissioned providers.
• Support transitions to adulthood with appropriate technology and equipment.

Job description

A Social Worker is required for work within the Adults Transition Team in the vibrant city of Bristol. Excellent rates of pay up to £32ph. 3-month initial contract with possibility to extend. 37 hours per week. Flexible hybrid working arrangements.

The Young Adults Transitions Service are a citywide team who can work with young people with additional needs from age 14-25 to support them to be as independent as possible as they reach adulthood.

We complete Care Act assessments and support our young adults through occupational therapy support to maximize independent living. We may also complete support plans to support the commissioning of services to meet needs, and we will utilize other sources of non-commissioned support like charitable organizations and provide technology and equipment to maximize young people's independence.

You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK.
