Eden Brown Synergy is seeking an experienced Adult Social Worker to join the Adults Locality Team in the Vale of Glamorgan. The role offers a pay rate of £35.11 per hour, a 37-hour week, and an initial 3-month contract with flexible working.

You will assess needs, develop care and support plans, manage risk and safeguarding concerns, and support individuals to achieve greater independence and wellbeing, working collaboratively with health and other partners.

Qualificações

• SWE registration is required or preferred.
• Driver with access to own car is desirable.
• Must reside and work in the UK.

Responsabilidades

• Complete Care Act assessments and reviews.
• Develop person-centred care and support plans.
• Manage a varied and complex caseload.
• Undertake safeguarding enquiries and risk assessments.
• Work with health professionals, families and partner agencies.
• Support hospital discharge and transitions.
• Promote independence, choice, and strengths-based practice.
• Maintain accurate case recording and reports.
