Julia's House in Devizes is seeking a Transition Nurse (Band 6) to support children and young people with life-limiting conditions. You will manage a caseload, act as Named Nurse, and coordinate care plans centred on the individual needs and wishes of each family.

You will guide transition from children’s services into adulthood, work with schools, hospitals, and adult services, and promote advocacy, best interests decisions, and safeguarding.

Job description

Come and join our nursing team as a Transition Nurse and play a vital role in supporting children and young people with life-limiting and life-threatening conditions, including those preparing to transition from children's services into adulthood., As a Band 6 Nurse, you will hold a caseload of children and young people and act as a Named Nurse, ensuring the ongoing assessment, planning, implementation and evaluation of care. You will develop strong therapeutic relationships with families, providing continuity, coordination and advocacy whilst ensuring that care remains centred around the individual needs, wishes and aspirations of each child and young person.

A key aspect of this role is supporting young people through transition into adulthood. You will hold a dedicated caseload of young people in transition, supporting them and their families from approximately age 14 onwards. Acting as a key point of contact throughout the transition journey, you will coordinate person-centred transition planning, helping young people prepare for adult services whilst ensuring their goals, aspirations and changing needs remain at the heart of decision-making.

Working closely with colleagues across Julia's House and with external health, social care, education and adult service providers, you will help develop and embed effective transition pathways that improve experiences and outcomes for young people moving into adult services. You will provide specialist advice and support relating to transition planning, advocacy, the Mental Capacity Act, Best Interests Decision Making, Deprivation of Liberty Safeguards and Liberty Protection Safeguards, helping families navigate what can often be complex systems and services.

Applicants for this role must be a Registered Children's Nurse (RSCN/RN Child), Registered Learning Disability Nurse (RNLD) or Registered Adult Nurse (RN Adult/RGN) and hold a Diploma or Degree qualification with active NMC registration. You will have experience of working with children and young people with complex needs and be passionate about delivering high-quality, family-centred care.

This is an exciting opportunity for an experienced and compassionate nurse who is committed to improving outcomes for children, young people and families. Based at our Devizes Hospice, supporting children, young people and families in hospice, home and community settings. Regular travel will be required to schools, colleges, hospitals, adult hospices and partner organisations.
