Job summary

Julia's House in Devizes is seeking a Transition Nurse (Band 6) to support children and young people with life-limiting conditions. You will manage a caseload, act as Named Nurse, and coordinate care plans centred on the individual needs and wishes of each family.

You will guide transition from children’s services into adulthood, work with schools, hospitals, and adult services, and promote advocacy, best interests decisions, and safeguarding.

Qualifications

• Qualified nurse with active NMC registration.
• Experience with children and young people with complex needs.
• Commitment to family-centred care and advocacy.

Responsibilities

• Provide transition-focused nursing care for children and young people.
• Coordinate person-centred transition planning across services.
• Support families and advocate for the child throughout transition to adulthood.
