Job summary

Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust is seeking a Band 6 paediatric transition specialist for a 12-month fixed-term post, 25 hours per week. Based at Doncaster Royal Infirmary, you will work with a multidisciplinary team to provide seamless transition for young people up to 25 with complex long-term health needs.

• Current registration with NMC.
• Evidence of continued professional development (CPD).

• Lead transition plans and ensure timely referrals and continuity of care.
