Qualifikationen

• 3 years senior clinical experience at Band 6 or above.
• Independent Prescriber (or expected) and related regulatory training.
• Teaching and assessing qualification/equivalent.

Aufgaben

• Provide leadership and clinical governance for Care of the Elderly and Acute Frailty ACP service.
• Lead multidisciplinary teams and oversee care pathways.
• Perform advanced clinical decision-making and initiate appropriate investigations and referrals.
• Deliver high standards of care with emphasis on safety, dignity and patient experience.
• Support development of geriatric outreach and education within the trust.
