Zusammenfassung

Rainbows Hospice for Children and Young People is seeking either a qualified Advanced Clinical Practitioner (ACP) or a Trainee ACP on an ACP training pathway to join our multidisciplinary team. The role spans hospice, community and hospital settings, with an on-call rota and flexible working to meet service needs.

For the Band 8a ACP, a Master’s level ACP, non-medical prescribing and experience with complex paediatric cases are expected; trainees must be on a recognized ACP pathway and committed

Qualifikationen

• Qualified ACP with master’s level practice.
• Non-medical prescribing qualification.
• Experience in children’s palliative and end-of-life care.
• Ability to work autonomously across hospice, community and hospital settings.

Aufgaben

• Provide expert clinical assessment and symptom management for babies, children and young people.
• Deliver specialist palliative care across multiple care settings.
• Support progression towards ACP status for trainee ACPs with supervision and development opportunities.
• Work with a multidisciplinary team to optimise patient care and outcomes.
