• Provide specialist tissue viability advice and support to patients across community and inpatient settings.
• Undertake holistic assessments of patients with complex wound care and tissue viability needs.
• Plan, implement, and evaluate evidence-based care programmes to support wound healing and skin integrity.
• Empower patients and carers through education and shared decision-making to encourage independence.
• Liaise with multidisciplinary teams and other agencies to ensure seamless patient care.
• Support education, training and competency development for healthcare professionals across the Trust.
• Maintain professional accountability, NMC registration requirements, continuous professional development, and clinical supervision.
• Promote cost-effective use of resources and adherence to the Trust wound care formulary.
