Job summary

South Tyneside Council is seeking a caring, practical Reablement Worker to join the casual team at Borrowdale House. You will work with Therapy and Support Leads to deliver person‑centred reablement care and support plans for residents in South Tyneside.

Shifts include days, evenings, nights, weekends and bank holidays with short‑notice flexibility. Experience in adult social or health care is valued and you’ll help people regain independence after hospital stays.

Qualifications

• Experience in Adult Social or Health care.
• Ability to support people using personalised, outcome‑based reablement plans.
• Communicates clearly and works well with families, colleagues and other professionals.
• Builds positive relationships with a wide range of people.
• Makes sound professional decisions.

Responsibilities

• Deliver person‑centred care using a therapy led, individualised reablement plan and risk assessments.
• Assist with daily routines and help rebuild confidence.
• Administer medications where required.
• Collaborate with Therapy and Support Leads, Registered Manager and families.
• Work across days, evenings, nights, weekends and Bank Holidays with flexibility.
