Job summary

South Tyneside Council is recruiting casual Reablement Workers for Borrowdale House in South Tyneside. You will support people to rebuild independence after hospital stays, following therapy-led, person-centred care plans and carrying out risk assessments.

This role offers flexible days, evenings, nights, weekends and bank holidays. Prior experience in adult social care is preferred; DBS disclosure will be sought, and vaccination requirements may apply.

• Has experience working in Adult Social or Health care.
• Can support people using personalised, outcome-based reablement plans.

Responsibilities

• Support people with daily routines and rebuilding confidence.
• Follow therapy-led, individualised reablement care plans and risk assessments.
• Administer medication where required as part of care duties.
• Collaborate with families, colleagues and other professionals to ensure safe, person-centred care.
