An exciting opportunity has arisen for a Therapy Assistant Practitioner to join a forward thinking and dynamic Community Stroke Team within Heywood, Middleton, and Rochdale. We would welcome applications from an organised and motivated person that has experience of working with individuals post Stroke, either in a community or inpatient setting. The ideal applicant would be passionate about post-Stroke recovery and enthusiastic about rehabilitation to help Stroke survivors achieve their goals, works well as part of a team and can contribute to innovative ideas on clinical pathways and can help drive forward excellence. We are a dynamic and welcoming MDT where a supportive learning environment and continuous professional development is actively encouraged. Regular supervision and clinical support are provided both formally and informally. The team provide goal orientated Stroke rehabilitation within the community setting to both acute stroke presentations through their hospital discharge pathway and those with stroke specific long-term rehabilitation needs. You will need to be able to work flexibly, hold a full UK driving license and have access to a vehicle. This is a full-time position advertised at 37.5 hours per week however, applications from suitably experienced candidates who are seeking a minimum of 30 hours per week will be welcomed. Work as an active member of the Stroke Team assisting in the delivery of integrated rehabilitation in the Heywood, Middleton and Rochdale area, supporting qualified therapists (occupational therapists, physiotherapists and speech and language therapists), and specialist nurses in assessing and treating individuals post stroke. Support implementation of therapy-specific programmes devised by qualified staff that works towards the individual's goals. This may be intensive, daily or weekly and may include multiple therapy-specific programmes within a treatment session. Support social inclusion as part of rehabilitation that promotes independence and improvements in health and well-being. The post holder will:

  • Use technical and creative skills in the provision of therapeutic activities for individual patients and will be responsible for evaluating and amending treatment programmes and interventions within guidelines and competency frameworks supervised by the qualified staff.
  • Will take responsibility for their own workload within the framework laid out by qualified staff.
  • Will be responsible for delivering defined therapy input without direct supervision and will have access to a therapist for advice and direction as required.

The work will take place in a variety of settings including, in-reach to hospital, patients' own homes and other community settings often as a lone worker.
