Job summary

West Suffolk NHS Foundation Trust is seeking an Assistant Practitioner to join the Suffolk Integrated Stroke Service within the Stroke Early Supported Discharge Team. You will work with therapists to deliver evidence-based rehabilitation to stroke survivors in their homes and communities.

You will manage a delegated caseload, supporting assessment, treatment and ongoing review of patients with non-complex needs, while contributing to six-month stroke reviews, service development and SSNAP data

Qualifications

• Foundation degree in healthcare is required or equivalent.
• GCSE Maths and English at grade C/4 or above.
• Demonstrated intermediate theoretical knowledge and clinical reasoning from coursework and training.

Responsibilities

• Provide skilled rehabilitation support within the Stroke ES Discharge Team, delivering assessment, treatment and progression of evidence-based rehabilitation programmes for stroke patients with non-complex needs.
• Work independently with delegated patients under supervision of registered therapists, monitoring outcomes and adapting treatment plans.
• Support patients to maximise functional independence using evidence-based interventions reflecting patient abilities and preferences.
• Contribute to six-month post-stroke reviews and maintain clinical and audit data, including SSNAP database input.
• Support therapists, supervise students where appropriate, and participate in delivery of a seven-day service.

• Provide skilled rehabilitation support within the Stroke Early Supported Discharge (ESD) Team, delivering assessment, treatment and progression of evidence-based rehabilitation programmes for stroke patients with non-complex needs.
• Work independently with delegated patients under the supervision of registered therapists, monitoring outcomes and adapting treatment plans in line with agreed protocols and person-centred goals.
• Support patients to maximise functional independence using evidence-based interventions that reflect their individual abilities, preferences and cultural needs.
• Contribute to the delivery of six-month post-stroke reviews and maintain accurate clinical and audit data, including inputting information to the Sentinel Stroke National Audit Programme (SSNAP) database.
• Provide support to therapists, supervise students where appropriate, and participate in the delivery of a seven-day service.

• Foundation degree in healthcare, or equivalent
• GCSE Maths and English Grade C/4 or above
• Can demonstrate intermediate level of theoretical knowledge and clinical reasoning skills acquired through coursework, in-house training and experience, Experience as a Therapy assistant or experience at a similar level in a related field of health care / health and fitness sector
• Demonstrable experience of delivering tailored personal care with an associated level of senior responsibility
• Knowledge of medical conditions and functional needs within the context of stroke and neurological deficits.
• Completion of Integrated Therapies assistant core and/or specific competencies
• Ability to work on own initiative without direct supervision
• Knowledge of specific treatment protocols for neurological conditions
• Able to maintain clinical records and complete basic reports.
• Ability to provide and receive complex and sensitive information
• Ability to exchange condition related information with MDT, patients and carers
• Ability to evaluate patient's condition and context, subsequently providing necessary basic/simple needs -led assessment and planned interventions/care programmes, and identify functional needs for safe discharge.
• Health & Safety and risk awareness
• Able to evidence ability to plan, prioritise and maintain own clinical caseload.
• Knowledge of role of therapy in Acute and Community Care.
• Able to demonstrate role and duties to MDT and community colleagues.

• Experience of working across and liaising with variety of teams

• Understanding of the role and application of occupational therapy, physiotherapy, speech and language therapy and psychology.
• Knowledge of occupational therapy, physiotherapy, speech and language therapy and psychology within the Acute Care or Community care settings.
• Experience of group work with patients.
• Ability to plan and deliver range of prescribed interventions.
• Ability to prescribe and implement care programmes to facilitate timely discharge for patients.
• Ability to support specialist OT / PT input to care programmes for complex patients as directed
• Understanding of the issues around hospital throughput especially around admission avoidance and early discharge
• Understanding of reablement in the community setting, Good communication skills to overcome barriers of understanding particularly with adults with disabilities
• Good written and verbal communication skills Organisational ability and time management of self
