Job summary

The Dare2Dream Foundation is recruiting a Mental Health and Wellbeing Practitioner to support vulnerable children and young people across Abingdon and surrounding Oxfordshire. This term-time role delivers bespoke outreach interventions in homes, schools and community settings, assessing needs and developing tailored support plans.

You will work closely with families and partner agencies to achieve positive outcomes, with regular travel required to reach clients during 41 weeks per year.

Responsibilities

• Support vulnerable children and young people to improve their mental health, emotional wellbeing and life opportunities through bespoke outreach interventions.
• Deliver 1:1 support programmes in homes, schools and community settings, assess individual needs, develop tailored support plans, and work closely with families and partner agencies to achieve positive outcomes.
• Travel across Abingdon and surrounding Oxfordshire areas; field-based work with regular travel to reach clients.
