Radis Community Care has an opportunity for a Team Leader to join our team based in Clarence Park Extra Care in Malvern.

About the role

As a Team Leader, you will be the first point of contact for residents and staff onsite, as well as visiting health professionals. Your responsibilities include:

1. Leading the care shift and managing emergencies
2. Creating and updating Care Plans, Risk Assessments, and key documentation

Additional duties include:

1. Auditing documentation such as MAR Charts and Daily Diary Records
2. Completing staff rotas to ensure adequate staffing
3. Conducting quality monitoring checks like spot checks and reviews
4. Liaising with health professionals such as Doctors and District Nurses

About You

• Willing to work a mix of early shifts, late shifts, and alternate weekends
• Willing to participate in an on-call rota
• Minimum of 2 years' experience in the Health & Social Care sector
• A caring individual passionate about providing outstanding service
