Field Care Manager - Gloucestershire/South Wales

We are seeking a passionate and dedicated Field Care Manager to oversee the delivery of high-standard home care services in Gloucestershire and South Wales.

Main duties of the job

As a Field Care Manager, you will conduct client assessments, manage care plans, and ensure our services meet CQC regulations. The role involves regular travel to clients' homes and may include some evenings and weekends. You will liaise with the operational team, address client and family concerns, and collaborate with healthcare professionals to ensure holistic care.

Job responsibilities
  • Develop and review personalized care plans.
  • Liaise with the operational team regarding performance issues and training needs.
  • Ensure services meet CQC standards, conduct audits, and maintain accurate records.
  • Build relationships with clients and their families, address concerns, and update care needs.
  • Collaborate with healthcare professionals to ensure holistic care.
Requirements
  • NVQ Level 3 or 4 in Health & Social Care (or equivalent).
  • Experience in a supervisory/management role within health and social care.
  • A passion for delivering high-quality care, with exceptional communication and organizational skills.
  • Full UK driving license and ability to travel using own car.
  • This role requires travel around Gloucestershire and South Wales, including areas such as North Wales, Liverpool, Manchester, and Stoke.
  • Salary: £27,000 per annum, depending on skills and experience.
  • Remote/field-based role.
  • Working hours: 9am-5:30pm, Monday to Friday, with some flexibility over weekends and evenings.
  • Annual leave: 25 days plus bank holidays.
  • Ongoing training provided.

Qualifications
