Job summary

Creative Support in Blackpool is seeking an experienced Team Leader to guide learning disabilities supported housing schemes and domiciliary services. You will lead staff, provide supervision, coaching and performance management to deliver person-centred support that promotes independence and wellbeing.

You will coordinate staffing, respond to referrals, conduct initial assessments, and ensure co-produced plans with dedicated Key Workers.

Qualifications

• Experience leading a team delivering learning disabilities services.
• Ability to supervise, coach and manage performance of staff and volunteers.
• Commitment to person-centred planning and safeguarding.

Responsibilities

• Oversee deployment of staff to meet individual needs within resources.
• Lead initial assessments and ensure co-produced support plans.
• Organise regular person-centred reviews with input from service users.
• Collaborate with local multi-disciplinary teams and monitor quality.
• Maintain high standards of safety, documentation and KPI reporting.
