St Anne’s Community Services in Hull is seeking a dedicated Team Leader for Hull DCA to lead a busy care team providing person-centred support. The role is full-time (37.5 hours per week) with a salary of £26,780.12 per annum, rising after probation.

Qualifications

• Minimum 2 years’ experience within the care industry.
• Experience with the creation and implementation of care plans.

Responsibilities

• Effective Time Management: diaries and administration.
• Teamwork and Adaptability: work with managers and staff, cover tasks as needed.
• Care Plan Management: ensure care plans are accurate, reviewed and risk assessed.
• Supervisions: conduct supervisions and annual appraisals.
• Spot Checks: carry visits and record them.
• Safeguarding: report concerns to management and external agencies.
• Maintain Care Management System (Access).
• KPIs measured against quality and standards.
