Job summary

Candlelight Care are looking to recruit a new Team Leader for the Glastonbury branch. The role involves evaluating and assessing new client packages, developing care plans, carrying out risk assessments and reviews, liaising with the Registered Manager and clients, and supporting our team of Care & Support Workers.

You will also be on the on-call team on a rotational basis and expected to attend regular Team Leader Meetings; hours include evenings and alternate weekends and require adaptability

Qualifications

• Diploma level 2 in Health & Social Care or equivalent.
• Ability to work without supervision.
• Confidentiality and caring approach.
• Full clean driving licence and access to own transport.

Responsibilities

• Assess new client packages and develop care plans.
• Carry out risk assessments and review existing packages.
• Support our team of Care & Support Workers and conduct staff observations.
• Be part of the on-call team on a rotational basis.
• Attend Team Leader Meetings regularly.
• Liaise with health and social care professionals.
