ABOUT THE ROLE

Your role as Team Leader involves supporting the home management team to deliver high-quality, person-centered care by creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Additional responsibilities include:

  • Leading the person-centred care planning process by implementing, monitoring, and conducting monthly evaluations, completing care plan audits, developing action plans, and being responsible for executing these plans.
  • Assisting in the smooth discharge process, including documentation, medication management, and keeping residents' next of kin fully informed where appropriate.
  • Monitoring the well-being of each resident, being alert to any changes impacting care, and ensuring that care plans are updated accordingly.
  • Practicing safe systems of work across various tasks, including moving and handling residents, assessing risks, and ensuring safety for everyone involved.
ABOUT YOU

The ideal candidate will have:

  • An NVQ Level 3 or equivalent in care for care homes.
  • At least two years’ experience in a senior role working with residents.
  • Effective communication skills, both verbal and written.
  • Leadership ability to manage staff and act responsibly in the absence of the manager.
