Job summary

Nottinghamshire County Council is seeking a Targeted Practitioner to work flexibly across services, supporting disabled children and young people countywide. You will back leadership teams with bespoke assistance and help families navigate challenges to access resources.

The role focuses on delivering targeted interventions, collaborating with coordinators, and engaging with stakeholders to promote child and family well-being across the county.

Qualifications

Responsibilities

• Deliver targeted interventions and support for disabled children and young people.
• Collaborate with co-ordinators to ensure the smooth running of our countywide service.
• Provide direct, tailored support to children and their families, helping them navigate challenges and access resources.
• Engage with various stakeholders to promote the well-being of children and families.
