Job summary

Creative Support Ltd is seeking an enthusiastic and experienced Supported Living Manager to join our service in Telford. You will work with regional managers to ensure delivery in line with needs, contracts, and quality standards, developing outcome-based support plans to promote independence and life skills.

You will lead a dedicated team, promote equality and diversity, and collaborate with community partners, volunteers, and statutory services.

Qualifications

• Experience leading and managing teams.
• Good understanding of learning disabilities, mental health and autism.
• Ability to promote recovery and positive engagement.

Responsibilities

• Lead and manage a team to deliver person-centred support.
• Assess needs and plan transitions; develop outcome-based support plans.
• Collate data and write assessments, reports and support plans.
• Promote independence and tenancy sustainment.
