Job Description

Permanent contract | £27,711 to £32,654 per annum (Scale F/G) | 37 hours per week | St James Health Centre, Kings Lynn

We have an exciting opportunity for an Assistant Practitioner to join our enthusiastic team within the West Norfolk Home First Hub Social Work Team. We work alongside a multidisciplinary team of community health colleagues and hospital discharge team. The Home First Hub primarily supports older people and adults with physical disabilities who are ready to be discharged from hospital but need some support to do so, or who need some extra support to avoid being admitted to hospital.

Key Responsibilities
  1. Undertake Care Act Assessments to facilitate hospital discharges or prevent hospital admissions.
  2. Carry out assessments in a strengths-based approach to promote independence.
  3. Develop Support Plans tailored to individual needs, including Direct Payments and Personal Budgets.
  4. Consider Carers Assessments for informal carers.
  5. Participate in the team’s duty rota to support the service’s duty functions.
Candidate Requirements
  • Experience of undertaking assessments in social care and knowledge of the Care Act 2014, including Safeguarding Adults, is highly desirable.
  • Ability to work independently and as part of a team.
  • Excellent communication and recording skills.
  • Ability to undertake assessments in a fast-paced environment in a timely manner.

This position is office-based at St James’ Clinic, King’s Lynn, with occasional hybrid working possible with prior agreement.
