Job summary

Real Life Options is seeking a Support Worker to join the Outreach and Supported Living team covering Redcar and surrounding areas. You will provide personalised support to adults with autism, mental health, and learning disabilities, helping with daily living activities and promoting independence.

The role involves shift work including evenings and weekends on a rota, with DBS checks and training provided. A driver is desirable but not essential, and a positive attitude is valued.

Responsibilities

• Provide personalised support to adults with autism, mental health and learning disabilities in their homes.
• Help with daily living activities and promote independence.
• Assist with meal preparation and household tasks.
• Develop and implement individual support plans based on client needs and goals.
• Encourage community involvement, social interaction, and skill development.
• Monitor and document client progress and report changes to the team.
