Job summary

Active Care Group in Falconwood, London is seeking a dedicated Support Worker to help a gentleman with a severe brain injury and loss of vision. You will provide person-centred daily living support at home and in the community, and assist with appointments and activities to promote independence.

The role features a two-week rota with day shifts and sleep-in nights, averaging 35 hours per week. A full UK driving license, flexibility and reliable teamwork are essential; full training is provided.

Qualifications

• Full UK driving licence held for at least 12 months.
• Excellent communication and interpersonal skills.
• Reliable, flexible and able to work independently or in a small team.
• Experience in care or support roles is beneficial but training will be provided.

Responsibilities

• Provide person-centred support with daily living activities at home and in the community.
• Accompany the client to appointments and social activities to maintain independence.
• Drive the client's Motability vehicle and coordinate with family and support network.

You'll play a key role in helping him maintain his independence, participate in activities he enjoys, and live life to the fullest. **What you'll be working:** This full-time position averages 35 hours per week and follows a two-week rota pattern, including a mixture of day shifts and sleep-in nights. The team generally works 24-hour shifts, with day shifts running from 6:00am to 10:00pm and sleep-in support provided overnight. **What you'll be doing:** You'll provide person-centred support with daily living activities both at home and in the community. This includes accompanying the client to appointments, social activities and outings, supporting his independence, and helping him manage his daily routine.
