Qualifications

• Provide daily support to adults in residential services.
• Deliver person-centred care focusing on independence, dignity and inclusion.
• Be flexible with shifts, including nights and sleep-ins.
• Work collaboratively with families, healthcare professionals and external agencies.

Responsibilities

• Deliver high-quality, person-centred support tailored to individual needs.
• Assist with daily living activities, personal care, health and wellbeing, budgeting, and accessing community opportunities.
• Follow care plans, risk assessments, PBS plans and safeguarding procedures to ensure safe support.
• Monitor and report changes in health, wellbeing, behaviour, or support needs and maintain records.
