Job summary

Connected Health Group is recruiting a dedicated Support Worker to assist adults with learning disabilities and complex needs in a supported living setting in Northern Ireland. You will follow personal care plans, perform health checks, and work solo when required, while promoting independence and dignity for service users.

The role offers training, development opportunities, and a supportive team environment within Connected Living, a division of Live Connected.

Qualifications

• Minimum of 6 months' experience in Supported Living Services.
• Must be based in Northern Ireland.

Responsibilities

• Support service users with daily living tasks in line with care plans.
• Follow individual care plans and risk assessments.
• Complete health and safety checks.
• Lone work as per risk assessments.
• Safely administer and order medications.
• Document daily notes and handovers by shift end.
• Collaborate with the MDT to support wellbeing.
