Job summary

Connected Health is actively seeking a dedicated Support Worker to assist adults in supported living settings in Northern Ireland. You will help with daily living tasks, adhere to care plans, and ensure safe medication handling while working with a multidisciplinary team across services.

The role emphasizes compassionate care, independence for service users, and alignment with organisational policies and documentation practices.

Qualifications

• Minimum of 6 months’ experience in Supported Living Services.
• Must be based in Northern Ireland.
• Support service users with daily living tasks in line with individual care plans.
• Complete health and safety checks in line with organisational policies.
• Confidently lone work in accordance with lone working risk assessments.
• Ensure safe administration and ordering of medication.

Responsibilities

• Support service users with daily living tasks as per care plans.
• Follow Connected Living policies and procedures.
• Follow individual care plans and risk assessments for each service user.
• Complete daily notes, handovers, and required documentation.
• Deliver clear, accurate handovers to incoming staff.
• Collaborate with MDT to support wellbeing.
