Job summary

700 Club in Darlington seeks an experienced Support Worker to deliver tailored, person-centred support that helps individuals and families live independently. You will manage a caseload, prepare support plans and reports, and liaise with partner agencies to coordinate advocacy and support.

You will assist with budgeting, maximising income, tenancy responsibilities and daily living skills, while promoting healthy living and community participation to reduce isolation.

Qualifications

• Minimum of one year's frontline support experience.
• Experience supporting vulnerable individuals and families is essential.
• Ability to prepare support plans and reports and liaise with partner agencies.
