Job summary

British Red Cross in Scotland is seeking a flexible Support Worker for a Discharge to Assess role in South Ayrshire. You will respond to referrals, visit service users at home, and provide practical and emotional support to help people regain independence.

The role focuses on preventing hospital readmission, coordinating with NHS, social care, and housing professionals, and creating tailored support plans. A registration with the SSSC within 3 months is required, and the position is casual with

Qualifications

Experience supporting people with physical, sensory, or mental health needs.
Strong communication skills and the ability to work both independently and in a team.
Knowledge of NHS and social care services and how to access them.
A flexible approach to working hours, including evenings and weekends.

Responsibilities

Visiting service users in their homes to assess needs and create tailored support plans.
Assisting with daily living activities such as bathing, dressing, and meal preparation.
Liaising with health, housing, and social care professionals to ensure consistent support.
Carrying out welfare checks and helping individuals rebuild their independence.
