Job summary

Hillcrest Futures is recruiting for a Support Worker to join a dedicated team providing high-quality care to four individuals, following their Personal Outcome Plans. The role involves direct support, personal care, PEG feeding knowledge, medication administration, and collaboration with stakeholders to meet individual outcomes.

Applicants must have a full UK driving license, be prepared for shift work including early mornings, late shifts, sleepovers (enhanced rate) and night shifts, and uphold

• Provide direct support to individuals with profound Global Developmental Delays and Physical disabilities, as agreed in individual Personal Outcome Plans.
• Provide intimate personal care.
• Have knowledge and or understanding of PEG feeding.
• Provide full support with medication dispensing and administration.
• Undertake case co-ordination responsibilities with, and on behalf of individuals and be an effective team member.
• Work effectively with all stakeholders and mentor and support the induction of new staff whilst contributing to team learning and development.
• Contribute to service improvements and support the line manager to ensure that the individual needs of individuals are met through outcome planning, reviews, and ensure that relevant outcomes plans are in place.
• Undertake shift work as required by the service, earlies, lates, sleepovers (paid at an enhanced rate) and nightshift duties.
• Have a full UK Driving licence.

About the Service

The Support Worker will work as part of a team to deliver a high standard of care and support to four individuals to help them to achieve their agreed individual outcomes. As a Support Worker you will be expected to adhere to the standards set out by the Scottish Social Services Council Codes of Practice and Hillcrest Futures Policies and Procedures.

About the Role
