Job summary

Qualifications

• Previous experience working with adults with mental health needs and learning disabilities.
• NVQ Level 2/3 in Health and Social Care or equivalent is desirable.
• Full UK driving licence is required.
• Enhanced DBS checks will be completed prior to employment.

Responsibilities

• Key work one or more individuals, meeting weekly to provide support within professional boundaries.
• Support people with mental health difficulties in line with their support plan.
• Assist with managing tenancies and moving home where required.
• Provide personal care as needed per support plans.
• Support health needs including medication administration and appointments.
• Maintain accurate records and reports in line with standards.
• Attend team meetings and contribute to service development.
• Safeguard vulnerable individuals and escalate concerns as appropriate.
